Looks like Kilsarin's Inish pavers. Lots of people make similar ones but they are normally tumbles to try and look like old clays
Kilsarin are an Irish company but supply some larger public projects in GB.
You can contact them to check if its a public project they will have it recorded.

meant to say with that texture, there are a couple of other companies who make blocks that size but not with the smooth texture
