I'm no expert but that looks like quite a unique piece. My guess would be as you suggest a comission or alternatively, repurposed architectural salvage. I used to use a rec yard in Surrey in a place called Wood Street Village near Guildford. He used to have all sorts of interesting stuff. You could try him if he is still going. His no. is 01483 235536. Alternatively you could look to have a stone quarry make you one up. i am in Shropshire now and we have one that can make you up stuff but it won't be cheap. Just a thought, you could possibly look to get one made up in metal by a blacksmith. There are some good local artisans around Surrey. Good luck.

Sarah, as you’re Farnham/Surrey based it’s worth knowing about a smaller reclamation yard on the A325 at Wrecclesham, Farnham (just up the road from me), if you don’t already....
Not as expansive as wood street, focusing on internal design items but has ever changing stock including ironworks, gates paving etc.

It’s 1/2 mile up the road from London Stone’s brand new showroom ( which is worth a look if you’re design based )
If you’ve got time, a further mile down the road is Garden Styles new nursery specialising in specimen and exotic plants ( it’s next to Forest Lodge Garden centre which is good to grab a coffee or bite to eat ....not that we ‘ever’ do that ;)
In fact, we’re becoming a ‘must’ visit area !!!

As Edward said getting something made in steel would be a viable alternative.
When I was a lad there was a local firm making concrete products, when my Dad asked about getting some coping made for a wall they said “you’re a carpenter, make some moulds and we will fill them”, which he did.
Therein lies the problem with concrete, you need someone to make and fill the moulds. Not impossible, but maybe harder than you think.
