Phil, bare in mind it must be "your" companies RA, you cant just send an example one plucked from the internet. You need to edit out everything that is not relevent and insert everything that is relevent to your operations. You need to make documents specific to your business and operations. They will / may ask you for further proof of anything / operations your business carries out. This document is a good start though.
For instance where it comments on working on steep embankments with rope and harness, they may ask you for your "fall arrest harness and working at heights" training cert. If you dont have the certs for certain operations to back up your application, they won't pass you on it. Just stuff like that to watch out for when your editing it and making it your own doc.

I don't think it's a good sign you just want to get a template and 'change' it to qualify you as a safe contractor business. You would have a far better robust and thorough document if you CREATED it instead, plus a genuine understanding of the content. I am 'just' a sole trader as well but I did it last winter and got accredited first time. I know my H&S docs inside out and think things through/inside out when it comes to aspects of work that relate to them.
