Have you tried tying a long board across the top of the ladder ? Not ideal and it depends on what you are cutting but on the rare occasion I've had to do this , its easier not to do these sort of jobs .
I agree ,encountered similar problems cutting at height even with my henchman so decided to put a limit on the hedge height I was prepared to undertake .
I would just say height is beyond my remit and more within the scope of a tree surgeon .
There is more than enough accessible hedge cutting rather than having to scale heights ..
my thoughts too................... plenty of other work about so just say you can't do it.
