Oh dear - poor chap.
We've all take risks and I dare so will continue to do so. It's taking a Risk Assesment of each procedure that is so important. Most lessons come with experience and these can be passed on.
A sharp lesson was bought home to me many years ago when we were landscaping a frontage and left our 45gall water butt overnight near the footpath, with about 2ft of water in it. A young boy climbed onto the heap of ballast, tried to reach the water and toppled in. Luckily a passer by saw his feet sticking out of the top and hauled him out, otherwise the unthinkable would have happened.
