1. They do not always know why they are successful - a good mgt review meeting will force them at least once a year to review the reasons why they were good and bad!
2. Having procedures and not documenting them means that things change!
3. Large companies are probably more at fault here for they have good systems in place, guy moves and new broom comes in and sweeps away all systems whether good or bad, seen it no end of times!
It is always enjoyable taking companies through to ISO
What I do is like you say, design their own systems - they need to see the benefits of running the system, there are bits associated with Training and competencies which can be a drag for a small company re keeping records etc but in general all the companies I have installed ISO on have continued with the exception of ATAT who recently canceled!!
